Incident Reports and Blog Reminder

Greetings,

A quick reminder to all Central Services staff to be sure to periodically check the Chief of Central Services blog for important information. The address is: http://cenchief.blogspot.com/

I will also share one important piece of information that is included there concerning Incident Reports.

To facilitate filing and finding incident reports throughout the system, HPL is adopting a naming convention for incident reports.

Please name all incident reports in the following manner:

Date-location-staff initials-subject-name of person involved.

For example, if, yesterday, staff member Mike Van Campen at Central filled out an incident report about customer Martin Smith assaulting the security guard it would be named:
080409-CEN-MVC-Assault-Smith.doc

If you don’t have a person’s name, just leave it off.
All reports will be able to be viewed on the G:\ drive in the G:\INCIDENTS files.
